Missouri Handicap Parking Permit Application
Residents of Missouri who have disabilities that limit mobility may apply for a handicap parking permit through the Missouri Department of Revenue (DOR). These permits allow individuals to park in designated accessible spaces to improve convenience and access to services.
Types of Handicap Parking Permits in Missouri
Missouri offers both permanent and temporary disability parking permits:
- Permanent placards are valid for up to four years.
- Temporary placards are valid for up to 180 days.
- Disabled license plates are also available for individuals who own a vehicle and qualify for permanent disability status.
Who Qualifies for a Permit
To be eligible, an individual must be certified by a licensed healthcare provider as having one or more of the following conditions:
- Inability to walk 50 feet without stopping due to a physical condition
- Use of a wheelchair, cane, brace, crutch, or prosthetic device
- Lung disease that restricts breathing
- Heart conditions classified as Class III or IV by the American Heart Association
- Legally blind
- Other conditions that severely limit mobility, as verified by a healthcare professional
How to Apply
Applicants must complete the Application for Disabled Person Placard or Plates (Form 2769). This form is available at any Missouri license office or can be downloaded from the Missouri DOR website.
Steps to Apply:
- Complete Form 2769 – The applicant and a licensed physician, chiropractor, optometrist, podiatrist, or advanced practice registered nurse must complete and sign the form.
- Submit the Application – Forms can be mailed or submitted in person at any Missouri license office.
- Pay Applicable Fees – Temporary placards cost $2. Permanent placards and plates are generally free, though standard registration fees apply for plates.
Renewal Process
- Permanent placards must be renewed every four years. A new physician’s certification is not required unless the original certification was temporary.
- Temporary placards require a new application and physician’s certification each time they are renewed.

Replacement of Lost or Stolen Permits
If a placard is lost, stolen, or damaged, it can be replaced by submitting a new Form 2769 and checking the "replacement" box. A replacement fee may apply.
