Essential DMV Forms for Hawaii Residents

Whether you're applying for a driver's license, registering a vehicle, or updating personal information with the Hawaii Department of Transportation (HDOT), having the right forms is essential. Vehicle Registration Forms

  • Application for Registration of Motor Vehicle (Form CS-L(MVR)1)
    Used to register a newly purchased vehicle or a vehicle brought into Hawaii from out of state. Proof of ownership and a current safety inspection certificate are required.

  • Notice of Transfer (Form CS-L(MVR)50)
    Must be submitted within 10 days of selling or transferring a vehicle. It informs the DMV that you're no longer responsible for the vehicle.

  • Vehicle Title Application (Form CS-L(MVR)3)
    Needed when applying for a duplicate or corrected title. Be sure to submit the proper identification and vehicle details.

Commercial Driver’s License (CDL) Forms

  • Commercial Driver License Application (Form CDL-1)
